CohortWe investigated 21 PA patients who were admitted to the Third Department of Internal Medicine (Kagoshima University School of Medicine) and the Department of Respiratory Medicine (National Minami-Kyushu Hospital) from 1995 to 1999. They consisted of 17 men and 4 women with an average age of 59.7±12.2 years. The diagnosis criteria of PA was as follows: (a) a fungal ball on chest X-ray; (b) a suggestive chest X-ray (fungal ball or thickness of the wall of cavity or pleura) and positive culture; (c) a suggestive chest X-ray and positive serodiagnosis; and (d) (b) plus pathologic specimens of Aspergillus species. We excluded patients with other chronic inflammatory diseases, such as rheumatoid arthritis, cystic fibrosis and tuberculosis. All patients included in this study were negative for human immunodeficiency virus. None had any abnormality of the coagulation-fibrinolysis factors. Three Patients met the criteria item (a), 6 met those in (b), 1 met those (c) and 1 1 met those in (d). Six patients developed hemoptysis and were successfully treated (4 patients had lung resection with antifungal therapy while others underwent bronchial artery embolism with antibiotic therapy). These patients showed no evidence of bacterial infection. Fifteen patients without hemoptysis were admitted to our hospital due to an abnormal chest X-ray shadow.
